When you have new members join your chapter, you may want to give them a membership form to fill out. You should ideally get the following information for your records.

  • Full name
  • Address
  • Phone
  • Email
  • Birthdate
  • Any relevant allergies you need to know about (eg food)
  • Medical conditions you may need to be aware of (eg pacemaker, epilepsy)
  • Emergency contact info (who to call in an emergency).

A convenient way to do this is through a google form, where the members can enter their information, and it will all be kept in a spreadsheet for your records. If you’re not sure how to set up a google for, you can view a guide here www.aussiehatting.com/google-form
